The Lake View School-Based Health Center is a partnership between Advocate Illinois Masonic Medical Center and Lake View High School. The goal of the Health Center is to improve the physical and emotional health of Lake View students and teach them life-long positive health behaviors. Students must have a signed parental/guardian consent form on file before they can receive services at the Health Center.
During the 2006-2007 school year, the school-based health centers at Amundsen and Lake View High School provided more than 2,300 individual office visits and had over 7,000 student contacts through group visits. More than 90% of students who attend Lake View had a signed parental consent form on file.
Health Center staff are employees of Advocate Illinois Masonic Medical Center. Providers include Family Physicians, Nurse Practitioners, Clinical Psychologists, Health Educators, and a Medical Assistant. The Health Center is open Monday through Friday during school hours.
